We instituted a new way of processing checks, so we added a notice at the bottom of the 1^st page to explain the process. Also, we changed the address of the processing center listed in the instructions.

This IC expires October 31, 2007, however we are making this change so close to the expiration date because we need this form to send in a mass mailing before October 2007. The new information that was added to the form was needed before we send this form out.
