SSA recently became aware of the need to obtain OMB clearance of this information collection. The agency plans to use the existing stock of the form SSA-L707 until it is able to print the new version of the form with a valid OMB control number; SSA then plans to destroy the remaining stock of the form SSA-L707.

SSA has updated the format and information requested to include additional information to the custodian of the record to assist in identifying the record requested. The form also includes updated payment information and the addition of Privacy Act, Computer Matching, and Paperwork Reduction Act statements. Upon receipt of approval from OMB, a printing specialist will work on the margins and font size to create a readable form on two pages and will include sufficient space for credit card information and address lines.
