CMS has submitted and received OMB approval for the revised collection however the revised collection will require coded changes to our software applications that due to budget restraints and other policy issues cannot be incorporated at this time. This collection is vital in that it affects Medicare beneficiary entitlement and benefits. We would like to request continuous use of the existing approved collection (dated 06/04) until the software changes can be made. CMS will alert OMB once all systems software changes have been made for the approved collection of the revised CMS-2728.
