1. The instructions say that respondents should report on the number of graduates serving in primary care, however there doesn’t seem to be space on the application for reporting this information.
Response: This information was mistakenly missing from the application form and has now been added.
2. The instructions say that the application should state who the program administrator and authorizing official are. However, there doesn’t seem to be space on the application for reporting this information.
Response: The instructions have been revised to exclude this information from the application form. Grantees provide this information when they apply through Grants.gov on the OMB approved standard form 424. As a result, the program already has this information and does not require duplicate reporting.
The form requests information about the tuition for FT students and length of the program, however the instructions do not include this information.
Response: This information has been added to the instructions prior to the table on the last page.
