, the undersigned herein described as the herd owner, hereby agree to accept an indemnity
payment for the voluntary depopulation of swine originating from a herd known to be affected with pseudorabies. I also agree to present all
swine comprising the herd to animal health regulatory officials for removal to a facility for euthanasia and for disposal by either rendering or
by other means approved by the USDA and other applicable state authorities.

I further agree to follow the clean up and disinfection procedures described as follows:
