Revisions to the Form
Two minor non-substantive revisions were made to the form SSA-781.
The Privacy Act Notice was updated to include detailed language about the use of the information in computer matching programs and to specify the other agencies with which we may share this information.
The Paperwork Reduction Act Statement was updated to show the correct address to which comments may be sent.
