Instructions for 2005 Grantees
The Office of Safe and Drug-Free Schools, in conjunction with the Office of National Drug Control Policy is requesting your help in collecting information about the 2005 Grants for School-Based Student Drug Testing Programs. Please note that your participation in this survey is entirely voluntary.
As current grantees, we know you understand the important role drug testing can play in helping to curb youth drug use. We are collecting similar information for each of the three cohorts of student drug-testing grantees and we will use your responses to inform the public and members of Congress about the accomplishments of our grantees.
Although we intend to use the information only in the aggregate, please be aware that your individual responses may be subject to public disclosure under the Freedom of Information Act.
Please use the attached form to provide the information.
You may submit the information either by email to OSDFSDrugTesting@ed.gov or by hard copy Student Drug-Testing Questionnaire, 400 Maryland Avenue, SW, Room 3E324, Washington, D.C 20202. For information and assistance on completing the form, please contact Kandice Kostic by email at Kandice.Kostic@ed.gov, telephone 202-260-7836.
