Dear Ms. Bryant,

The United States Trustee Program would like to modify appendix E to the credit counseling application, OMB # 1105-0084. Most of the changes involve formatting changes along with a few additional questions that are posed to applicants. These additional questions are actually more specific sub-set questions from the prior appendix E that are necessary due to applicants’ failure to completely answer the questions on the prior appendix. The changes are as follows:

• Include contact person who can answer UTSP questions regarding appendix E

• Include agency number

• Require actual rather than estimated figures

• Specify which six month period is covered by the appendix

• Reformat the appendix to ask questions about debt management plans first and certificates second

• Replace first question of old appendix with questions 1 and 2 of new appendix

• Replace second question under debt management plan portion of old appendix with questions 5 and 6 on new appendix

• Replace sixth (last) question under debt management plan portion of old appendix with questions 9 and 10 on new appendix
