Justification of Changes made to 2070-0093 and 2070-0143:
EPA made the following new changes to the Forms R and A.
The box "WhereTo Send Completed Forms" number 2. APPROPRIATE STATE OFFICE (See instructions in Appendix F.) It should be in Appendix E. Appendix F is the EPA Regional Contacts, not the State Designated 313 Contacts.
In addition, we have revised the date on the bottom of the forms (both A & R) to reflect the effective date for the NAICS rule (i.e., August 7, 2006).
