This survey determines whether
returning OIF/OEF war veterans with loss of limbs and other very
severe and lasting injuries have access to the best of both modern
medicine and integrative holistic therapies for rehabilitation and
will ensure continuity of care for veterans who transition from the
DOD health system.
On behalf of this Federal agency, I certify that
