consistent with the following terms of clearance: OMB approves the
requested collection of information for purposes of gathering
information for program planning and development. In any reports of
results from this study, CDC will acknowledge the limitations of
the data for inferring impact of the program. OMB encourages
CDC/NIOSH to include additional mechanisms for non-response bias
analysis such as telephone follow up with non-respondents. CDC
should forward a description of these plans to OMB for inclusion in
the public docket prior to data collection. CDC shall also report
the achieved response rate and provide a summary of the results of
the nonresponse bias analysis to OMB upon completion of the
study.

The purpose of this project is to
evaluate the Fire Fighter Fatality Investigation and Prevention
Program (FFFIPP), and the effects of the FFFIPP recommendations and
information products which are periodically distributed to the
nations 30,000 fire departments.
On behalf of this Federal agency, I certify that
