(1) Unless the
two forms (sched. F and F-1) are stapled together, each shall
contain the control number. (2) The form(s) shall list the
consequences of failure to display a valid control number. (3) In
the next submission under this control number (but no earlier than
six months from clearance) DOT shall report on its consideration of
displaying up front in the instructions (a)where the completed
forms should be sent and (b) how questions may be answered. (4) BTS
is commended for its work on automating this information
collection. The next and all future submissions shall include
prints of screens and response alternatives used in the
collection.

The Bureau of Transportation
Statistics collects financial data from commuter and small
certificated air carriers to enable the Department of
Transportation to monitor the state of this important segment of
the airline industry.
On behalf of this Federal agency, I certify that
