The Centers for Disease Control and
Prevention (CDC) seeks review and approval of an automated
Management Information System (MIS) to maintain individual state
heart disease and stroke prevention information, and to provide a
consistent format for the information reported by these programs.
The proposed web-based MIS will employ a more formal, systematic
method of collecting progress reports and continuation
applications.
On behalf of this Federal agency, I certify that
the collection of information encompassed by this request complies
with 5 CFR 1320.9 and the related provisions of 5 CFR
1320.8(b)(3).
