VA must submit
the final version of directions for forms under this approval that
will be displayed on the VONAPP site as soon as possible but no
later than 6 months from the date of approval for this information
collection. Prior to the next request for OMB approval of this
package VA should reevaluate the necessity of requiring an
exemption to display the expiration date given that the forms will
be available online and the requirement to maintain a large
existing stock of paper based forms should diminish.

Surviving spouses, spouses, and
children of veterans send VA this form to apply for a change of
their program of education and/or a new place of training under 38
U.S.C. chapter 35. VA uses the information on the form to determine
the current eligibility of the claimant for educational assistance
for the new program of education or new place of training.
