Extension without change of a currently approved collection
The information gathered by these
forms (each form for a different age group), in combination with
other documented medical evidence, is used by State Agency
adjudicative teams to form a complete picture of a child's ability
to function. This information helps SSA to determine whether a
child is disabled, particularly in cases when a disability
determination cannot be made on medical evidence alone. The
respondents are applicants for Title XVI childhood disability
benefits and their adult caregivers.
On behalf of this Federal agency, I certify that
the collection of information encompassed by this request complies

1320.8(b)(3).
