FEMA Form 81-69, Comunity Assistance
Contact Report is used for telephone contact or brief visit with
NFIP communities to determine if program-related problems exist and
offer assistance. FEMA Form 81-68, Community Assistance Visit
Report is used to document a scheduled visit to NFIP communities
for the purpose of conducting a comprehensive assessment of the
community's flood plan management program and to assist the
community with NFIP and its requirements and implementing effective
flood loss reduction.
On behalf of this Federal agency, I certify that
the collection of information encompassed by this request complies
with 5 CFR 1320.9 and the related provisions of 5 CFR
1320.8(b)(3).
