protocol that is sponsored by CDC and will be collected at each
site. The other site-specific data collections are not covered by
the PRA. CDC will make the agreed upon revisions to the Tobacco
section of the Health Risk Behavior Survey and the race/ethnicity
categories of the VADTRS/SDQ Short Form. CDC will forward these
revised documents to OMB prior to fielding the survey. If the
lottery incentive structure is used at the Virginia site, CDC will
report to OMB the effectiveness of this incentive in boosting
response rate, and any problems encountered with administering the
incentive.

Population-based research in
Attention-Deficit/Hyperactivity Disorder (ADHD) projects consist of
three cooperative agreements to study youth ages 4-10 to determine:
1) the prevalence of ADHD in three defined communities, 2) rates of
comorbid conditions among those with ADHD, 3) describe health risk
behavior in ADHD youth, and 4) described current or previous
receipt of treatment for ADHD among school-age youth.
On behalf of this Federal agency, I certify that
the collection of information encompassed by this request complies
with 5 CFR 1320.9 and the related provisions of 5 CFR
1320.8(b)(3).
