Approved with
the changes faxed to OMB on Jan. 23, 2003. The proposed incentive
for this study is not approved. If funds become available and VA
determines that an incentive is necessary to achieve the desired
response rate, VA must submit a change request (83-C) and provide a
justification and detailed incentive structure at that time. Also,
VA will not ask for respondent social security number or full date
of birth.
