survey, NCHS schedules a debriefing with OMB staff. In addition, in
the future NCHS must work to coordinate and harmonize domestic
survey efforts with international initiatives to minimize U.S.
paperwork burdens and maintain high response rates for U.S.
surveys.

This study is a one-time collaborative
effort of Statistics Canada and CDC/NCHS to conduct a telephone
survey in both countries using the same questionnaire and study
design. Approximately 3000 adults will be interviewed in Canada and
5000 adults in the US. The questionnaire will cover chronic health
conditions, functional status and limitations, smoking, height and
weight, cancer screening, access to health care and demographics.
The project will be jointly funded with each agency covering the
costs of their own sample and splitting all other costs.
On behalf of this Federal agency, I certify that
the collection of information encompassed by this request complies
with 5 CFR 1320.9 and the related provisions of 5 CFR
1320.8(b)(3).
