Abstract: Commercial Carriers, Air Carriers, Evaluation Eligibility: This form is used to acquire information needed to make determination if commercial air carriers can support the DOD. Information is evaluated and used in the approval process. Failure to repsond renders commercial air carriers ineligible for contract award to provide air carrier service to the DOD.
