This submission describes proposed
activities of the NCHS Questionnaire Design Research Laboratory.
Survey specific research activities include questionnaire testing
and development for NCHS, CDC, and other Federal surveys. General
methodological research on the cognitive aspects of survey
methodology an studies of optimal design for the presentation of
health data will be pursued. Methods for data collection including
cognitive interviewing, focus groups, pilot household interviews
and Internet surveys.
On behalf of this Federal agency, I certify that
