Document the areal effects of
earthquakes. This questionnaire is designed to collect information
concerning the damage or degree of shaking from earthquakes. The
standard questionnaire is a postal questionnaire; we are also
experimenting with an electronic version of the questionnaire. Data
is used in mapping the extent of the shaking in terms of intensity,
is input to building codes, and is used for earthquake research.
Primary respondents to the postal questionnaire are local, State,
and Federal employees; some private postal questionnaires are sent
to private citizens if they request them to record their
earthquake.
