amended by and consistent with ED's memo to OMB of 7/26/99. In
addition, the following terms of clearance apply: (1) Following the
submission of the FFEL community version of a new PLUS loan
application, ED will work with the community and submit a new
version of the form that balances the needs and viewpoints of ED,
FFEL community, and the public at large. (2) ED, upon submission of
the 2001-2002 FAFSA, will report to OMB ED's decision on whether to
have the FAFSA serves as the PLUS loan application. If the decision
is to implement the FAFSA as the PLUS loan application, ED should
include specific timeframes in the report. If the decision is not
to implement, then ED should report its rationale. (3) ED will
amend the form to ask for the borrower's email address as an
optional item. (4) Consistent with ED's response #1 in the 7/26
memo, this approval is for two years.

This application form and promissory
note is the means by which a parent borrower applies for a Federal
PLUS loan and promises to repay the loan, and a school, lender, and
guaranty agency certifies the borrower's eligibility to received a
PLUS loan.
On behalf of this Federal agency, I certify that
