Abstract: The Commission adopted rules providing support for all telecommunications services, limited distance charges, and Internet access for all eligible health care providers. Health care providers who want to participate in the universal service program must file the following forms: FCC-465 to request eligible services; form 466 to certify that the most cost effective method of providing the services has been requested; form 467a to confirm the receipt of the requested services; and form 468 to ensure that the proper amount of universal service support has been calculated.
