This information
collection is approved through 12-99 as revised by the 6/30
addendum prepared by SSA under the following conditions: SSA shall
revise question 4D of the Payee Interview to solicit information on
individuals that have "provided substantial support and/or have a
significant involvement with the claimant." In addition, SSA shall
use existing data sources where possible in the survey for
information that is unlikely to have changed over time, such as
date of birth. Upon resubmission, SSA will provide an analysis of
the findings in this survey.

SSA is conducting a review of the
representative payee program. As part of this review, SSA is
proposing to conduct interviews with a sample of beneficiaries and
their representative payees. The information will be used to assess
the effectiveness of the representative payee program.
