is approved on the following conditions: CDC shall (1) revise
question one of the surveys to allow for self- reporting of race
and ethnicity in accordance with the revised standards on race and
ethnicity; (2) revise the burden statement to meet the requirements
under the Paperwork Reduction Act of 1995; and consider adding
response categories to question 3(h). CDC shall send to OMB copies
of the revisions prior to initiating the surveys. CDC's request to
provide a $5 payment is not approved.

Consumer satisfaction research is to
be conducted on a series of 11 information brochures developed by
the National Center for HIV, STD, and TB Prevention (NCHSTP). These
brochures focus on various opportunistic infections that affect
people with HIV and AIDS.
On behalf of this Federal agency, I certify that
the collection of information encompassed by this request complies
with 5 CFR 1320.9 and the related provisions of 5 CFR
1320.8(b)(3).
