The Federal Aviation Administration
(FAA), through the Office of Aviation Medicine (OAM), is
responsible for the aeromedical certification of pilots and certain
other personnel to ensure they are medically qualified to perform
their duties safely. In the accomplishment of this responsibility,
OAM provides a number of services to pilots and has established
goals for the performance of those services. This survey will be
designed to meet the requirements of the Government Performance and
Results Act of 1993 (GPRA) and the Executive Order No. 12862
"Setting Customer Service Standards" which mandate surveying
customers.
On behalf of this Federal agency, I certify that
the collection of information encompassed by this request complies
with 5 CFR 1320.9 and the related provisions of 5 CFR
1320.8(b)(3).
