collection is approved through 10-98 as revised by the October 1,
1997 HHS memorandum and under the following conditions: Upon the
next submission, HHS will succinctly outline the goals of the Rural
Health Network Grant Program; demonstrate how the reporting on this
program supports these goals, or if they do not, revise the
reporting to directly reflect the goals; and ensure that the goals
are measureable as required in the Government Performance Results
Act.

This request is for OMB approval of a
new reporting form that will be used to track the progress of
grantees under the Rural Health Network Grant Program authorized by
section 330A of the Public Health Service Act. The purpose of the
program is to assist in the planning and development of vertically
integrated networks of health care providers in rural
communities.
On behalf of this Federal agency, I certify that
the collection of information encompassed by this request complies
