The collection
is approved for three years, with the exception of the new 719B. We
received only one comment pertaining to this and other collections
under this OMB number (March 31 letter from Charter Industry
Services, Inc). We assume that problems identified are being
addressed and would appreciate brief report on this, especially
pertaining to having each Regional Exam Center using its own forms
and procedures. Also, with respect to the 719B, we are concerned
with the generality of the several questions in Part VI. For
example, the question whether you have ever used a dangerous drug
could be construed as overly broad. We would like to resolve our
concerns expeditiously, perhaps with a Conference call, involving
OST and the program office responsible for this collection.

The collection is necessary to
determine qualification of mariners for issuing licenses. OPA 90
mandates that individuals must give their consent for Coast Guard
to review NDR reports for issuance/renewal. Criminal offenses may
revoke credentials. OPA 90 mandate reduces terms of validity to 5
years.
