E.O. 12564 established this Federal
Workplace Drug Testing Program. These guidelines promulgate
certification standards for laboratories to conduct urine drug
testing and establish scientific and technical guidelines for drug
testing programs to assure compliance with the E.O.'s intent.
Included are the Drug Testing Custody and Control Form, the
National Laboratory Certification Program Application Form, the
National Laboratory Certification Program Inspection Report, and
associated recordkeeping requirements. SAMSHA is responsible for
administrative management of this multi-agency activity.
On behalf of this Federal agency, I certify that
the collection of information encompassed by this request complies
with 5 CFR 1320.9 and the related provisions of 5 CFR
1320.8(b)(3).
