OPM must modify
this form to to be in compliance with the requirements of 5 CFR
1320.8(b)(3). OPM's request to OMB certifies compliance with these
required disclosures but the information collection instrument and
instructions do not show how this is accomplished. Further, OPM
failed in its notice of 11/14/96 to comply with the requirement
that the notice specifically solicit comment on areas identified in
5 CFR 1320.8(d)(1). Because of these factors,OMB gives this form
approval for a limited time period. In its next request for OMB
approval should reflect adherence to these requirements.
