This collection
is approved in concept, on the condition that ETA addresses issues
of practical utility raised by OMB concerning the Job Corps policy
on HIV testing. Specifically, ETA shall explain (1) what additional
certification forms are required and how they are used in
conjunction with the Health Questionnaire to inform respondents
about the implications of testing; (2) policy regarding refusals
and whether any such incidents have occurred since the policy was
implemented; and (3) whether any individuals have been denied entry
into the program based on HIV or AIDS status. ETA shall provide
this information, along with an ICW, as soon as possible in order
to allow for full implementation of the form. In addition, ETA
shall, in the authorization section on page 2 of the questionnaire,
second sentence, insert the word (medical) prior to the word
(staff) to ensure that only medical personnel with a need for the
information can obtain access. Finally, this approval in concept
only covers the Health Questionnaire. The Child Care Certification
which was also previously approved under this OMB number must be
resubmitted under a seperate collection in order to be approved
under the Paperwork Reduction Act of 1995. The Child Care
Certification does not currently have OMB approval.
