This information
collection is approved through 9-97 under the following conditions:
ACF shall develop an information collection plan for the
implementation of welfare reform legislation. The plan shall
specify the information collections under IV-A child care and the
CCDBG programs to be eliminated. The plan shall be submitted to OMB
by November of this year, and will include a timetable for
completion of each activity.

States must use the form to fulfill
the requirements of section 402(i)(6) of the Social Security Act
with respect to child care services being provided. The form
collects service delivery information not available through other
mechanisms and funding information to determine the State's
compliance with the non-supplantation provisions found at section
402(i)(5)(D). By regulation, the States must report to the
Secretary on the items contained in the form no later than 90 days
after the end of the Federal fiscal year.
On behalf of this Federal agency, I certify that
