follow-up survey methodology that would enable it to achieve an 80%
response rate (the VA estimates that it will achieve a 65 - 70%
response rate.) This objective may necessi- tate telephone
follow-up in addition to mailings. No later than 10/96, the VA must
submit to OMB its plans for achieving a higher response rate and
analyzing the nonresponse bias; and 2) in the next submission for
OMB review, the VA provides an analysis of how self reported
responses in this instrument compare to medical records for PAP
smears for women, mammograms for women, and cholesterol level
ascertainments.

Congressional mandate requires VA to
annually report health promotion and disease prevention services
received by veterans. This mail survey will provide information
necessary to respond to that mandate.
