The FCC has
begun a rulemaking to make significant revisions to this collection
and wishes to extend use of this collection until the new form is
approved by OMB. FCC has permission to delete the OMB expiration
date as long as they can ensure that remaining stocks of this form
are deleted when the revised form is approved. Additionally,
several corrections were made regarding cost burden.
