The "Put Prevention into Practice" program developed materials to improve delivery of clinical preventive services. The proposed study collects data on service delivery and health behavior from providers, patients and medical records to evaluate the program in 1) community health care settings, 2) HMOs, and 3) private practice primary care settings.
