Abstract: THIS FORM IS USED TO RECORD INVESTIGATIVE DATA RELATING TO VEHICLE ACCIDENTS WHICH RESULT IN INJURY, PROPERTY DAMAGE EXCEEDING $10,000, OR DISABLING VEHICLE DAMAGE ON MILITARY INSTALLATIONS.
