-- ED shall delete the request for a job applicant's name from this
form. The data collected will be used for statistical purposes
only, for which individual applicant names are not necessary. -- ED
shall preprint the job announcement number, position title, and
date of application on the form, since this information will be
available to ED at the time the form is sent to applicants (with
the confirmation of receipt of an SF-171 for a particular vacancy).
-- ED has requested that this form be approved as an interim
collectio pending development of a government-wide policy and data
collection strategy in this area. As requested in ED's 10/8/93
memorandum to OMB one-year approval is granted.

FORMAT IN THE ANNUAL AAP REPORT TO THE EEOC.
