Abstract: OWNERS/OPERATORS MUST NOTIFY EPA OF CONSTRUCTION, MODIFICATIONS, STARTUPS, SHUTDOWNS, MALFUNCTIONS, DATE, AND RESULTS OF PERFORMANCE TEST. OWNERS/OPERATORS MUST RECORD WHEN EACH EQUIPMENT LEAK IS DETECTED, THE DATE EACH REPAIR WAS MADE, AND WHETHER OR NOT LEAKS WERE REDUCED BELOW 10,000 PPM AFTER EACH REPAIR ATTEMPT. SEMIANNUAL REPORTS OF EQUIPMENTS LEAKS MUST BE MADE.
