2/94 under the condition that AHCPR will provide additiona written
material pertaining to its sampling methodology, patient surve and
development of clinical decision models. As agreed to by Nancy
Pearce, OASH, supporting materials pertaining to the sampling
methodology and patient survey will be provided to OMB prior to the
fielding of the patient and physician instruments.

PREVENTION ACTIVITIES AND THE RELATED MEDICAL EFFECTIVENESS AND
OUTCOMES OF SUCH ACTIVITIES. PHYSICIAN PRACTICE PATTERNS WILL BE
