REGISTER ALL CHANGES OF LOCATIONS WITHIN A PREVIOUS 30-DAY PERIOD.
THIS IS TO IDENTIFY WHERE LIQUOR WAS SOLD BY THE DRINK AT LOCATIONS
OTHER THAN WHAT WAS LISTED O THE SPECIAL TAX STAMP ISSUED TO THE
CATERER. THE FORM IS FILED IN DUPLICATE BY THE CATERER, ALONG WITH
AN AMENDED ATF F 5630.5.
