Abstract: THE INFORMATION COLLECTED BY THIS FORM ALLOWS THE SOCIAL SECURITY ADMINISTRATION TO DETERMINE WHETHER OR NOT TWO UNRELATED INDIVIDUALS O THE OPPOSITE SEX WHO ARE LIVING TOGETHER ARE CONSIDERED MARRIED AND QUALIFY FOR SUPPLEMENTAL SECURITY INCOME (SSI) BENEFITS AS A COUPLE. THE AFFECTED PUBLIC IS COMPRISED OF APPLICANTS FOR AND RECIPIENTS OF S BENEFITS WHO HOLD THEMSELVES OUT TO BE MARRIED UNDER THE DOCTRINE OF COMMON LAW.
