FOR AN SBA DECLARATION THE GOVERNOR OF THE AFFECTED STATE MAKES A WRITTEN REQUEST STATING THE INCIDENT THAT OCCURRED, THE TIME AND PLACE AND CERTIFY THAT SBA CRITERIA HAVE BEEN MET.
