collection is approve for use until 2/29/91. In its next
submission, the PHS 6025-1 must include the following revisions: 1)
the burden estimate statement must be printed directly on the
application form or the instructions, and 2) the "General
Information" section which provides information on how to complete
the application package should be revised to incorporate
information for the applicant on the required certifications for a)
debarment and suspension, b) a drug-free workplace, and c)
lobbying.
