Abstract: THIS QUESTIONNAIRE IS DESIGNED TO PROVIDE FAST, LOW-COST INFORMATION COLLECTION CONCERNING THE EFFECTS OF EARTHQUAKE-RELATED TSUNAMIS OVER WIDE GEOGRAPHIC AREA. DATA COLLECTED ARE USED FOR MAPPING THE EXTENT AND TYPE OF TSUNAMI DAMAGE, AS INPUT TO BUILDING CODES, AND FOR EARTHQUAKE RESEARCH. PRIMARY RESPONDENTS ARE LOCAL, STATE, AND FEDERA EMPLOYEES (INDIVIDUAL CITIZENS ARE NOT QUERRIED DIRECTLY).
