PROVIDES SURVIVOR BENEFITS TO CHILDREN BETWEEN THE AGES OF 18 AND
22 IF THEY ARE UNMARRIED, FULL-TIME STUDENTS. THIS FORM IS PROVIDED
TO CHILDREN WHO APPEAR TO BE ELIGIBLE FOR BENEFITS WHEN THE DEATH
CLAIM IS INITIALLY RECEIVED.
