ARE REQUIRED TO FILE A MONTHLY REPORT. THIS REPORT SUMMARIZES ALL
TRANSACTIONS BY THE PROPRIETOR INCLUDING RECEIPTS, DISPOSITIONS AND
ON HAND QUANTITIES. ATF F 5220.4 (2140) IS USED FOR PRODUCT
ACCOUNTABILITY AND IS EXAMINED BY REGIONAL OFFICE
PERSONNEL.
