This form should
be cleared by GSA and not EPA. EPA should report its use of the
form and the associated burden hours to GSA. Any EPA-specif
modifications to the standard GSA form do need to be cleared by
EPA. EPA should submit to OMB in writing a list of all
grant-related information requirements and should identify which
are cleared by GSA or EPA, the appropriate OMB numbers, expiration
dates, and a statement indicating that EPA has examined its use of
GSA's standard forms, reported its use and burden hours to GSA, and
has reviewed each standa form to see if any EPA-specific
modifications have been made (and have been or will be submitted
fro OMB approval).
