Abstract: THE FINANCIAL WORKOUT PLAN WILL BE DEVELOPED AND SUBMITTED BY PHA'S WHICH HAVE BEEN DESIGNATED AS FINANCIALLY TROUBLED (FT) BY DEFINITION. THE FORM WILL BE USED ALSO FOR PHA'S DESIGNATED AS OPERATIONALLY TROUBLED (OT). THE QUARTERLY PERFORMANCE REPORT WILL BE USED BY BOTH FT AND OT PHA'S TO MONITOR PROGRESS AND EFFECTIVENESS OF WORK PLAN TASKS AND OBJECTIVES OF APPLICABLE PHA'S.
