THE INFORMATION COLLECTED ON THE
SSA-5031 IS NEEDED TO EVALUATE SSA'S IMPLEMENTATION OF THE
PROVISION TO TAX A PORTION OF SOCIAL SECURITY BENEFITS IN SOME
CASES AND TO DETERMINE TO WHAT EXTENT THE BENEFIT STATEMENTS SENT
OUT BY THIS AGENCY MET THE NEEDS OF THE BENEFICIARIES. IT WILL BE
USED FOR THOSE PURPOSES. THE AFFECTED PUBLIC WILL CONSIST O APPROX.
1,000 SOCIAL SEC. BENEFICIARIES WHO ARE SELECTED AT RANDOM TO
RESPOND TO THIS QUESTIONNAIRE.
